Private Health Fund Rebates


I am a provider for all major Health Funds.

If you have Private Heath Fund "Extras" cover, please check what your rebate will be. This varies considerably depending on which health fund you are a member of and what level of cover you currently have.

Medicare Rebates

I am currently a Medicare Provider, however there is a gap in fees for the initial consultation

especially.  Please enquire when booking. You are currently rebated $56.00 per consultation

from medicare if you have an Chronic Disease Management Plan (CDMP).

If you have a chronic condition your GP may choose to refer you under the "CDMP" This plan entitles you to 5 visits (in total) per year to allied health professionals involved in your health care. You do need to check with your GP as to whether you are eligible for this type of referral.
 
With this type of referral you are asked to pay up front and then claim the rebate from Medicare.
 

Veteran Affairs Provider

I am currently a Veteran's Affairs Provider. If you carry a gold card you will need to obtain a special referral from your GP called a D904, PRIOR to your initial consultation.

Please mention you are a DVA client when making your booking.
